CCBill Client Support Center - Client Account Change Forms
The forms linked from this pages are used to make changes to existing CCBill and CCBill EU accounts.
All forms need to be fully completed, printed, and signed. You may return the forms to CCBill by one of the following methods:
- Fax: Fax the signed form to 480.449.8801
- Email: Complete, print, and sign the form. Image the form (scan) and attach it to an email. Send the email to firstname.lastname@example.org.

|Account Authorization Form||An account owner can use this form to designate employees to be able to make certain changes to their account. Authorization is line-item and requires the signature of the account owner.|
|This form is used when a client wishes to cancel an account.|

|Address and Name Change||Use this form to make address and name changes to your account.|
|CCBill Change of Ownership||Use this form for change of account ownership for all US, Canada, Non-US and Non-EU Clients, whether High or Low Risk.|
|CCBill EU Change of Ownership||Use this form for CCBill EU accounts when ownership changes.|
|Change of Subaccount Ownership||Used when a subaccount is changing ownership.|
|Account Split Request||This form is used to split CCBill fees or partner payouts.|
|VISA Sponsored Virtual Vendor Account Cancellation||If you would like to cancel a VISA sponsored account, use this form.|
|Wire Transfer||Use this form to initiate wire transferred payments directly to your bank account, or to make changes to an existing wire transfer set up.|

Note: We are aware of an issue with some Acrobat Portfolios not displaying any content when viewed in certain browsers. This usually occurs if you are using Acrobat Reader 9 or earlier. If you open on of these PDFs and it does not display correctly, please right-click the link and choose "Save Target As" and save the form locally. You should then be able to open the form outside of your browser (in Acrobat Reader) and view the content.
Alternately, upgrading to Adobe Acrobat Reader X should fix the issue as well.
