Title   Description
Account Authorization Form | An account owner can use this form to designate employees to be able to make certain changes to their account. Authorization is line-item and requires the signature of the account owner.
Account Cancellation Form | This form is used when a client wishes to cancel an account.

Address and Name Change | Use this form to make address and name changes to your account.
US and Canada Adult Change of Ownership | Use this form for Adult accounts when ownership changes.
CCBill EU Change of Ownership | Use this form for CCBill EU accounts when ownership changes.
Change of Subaccount Ownership | Used when a subaccount is changing ownership.
Non-US Non-EU Change of Ownership | Use this form for ownership changes when the account is a non-US account and a non-EU account.
Non-Adult Change of Ownership | Use this form for ownership changes for non-adult accounts.
Account Split Request | This form is used to split CCBill fees or partner payouts.

Percentage Change Form | Use this form to take advantage of a special rate discount for clients hosting with Cavecreek Hosting.
VISA Sponsored Merchant Account Cancellation | If you would like to cancel a VISA sponsored account, use this form.
Wire Transfer | Use this form to initiate wire transferred payments directly to your bank account, or to make changes to an existing wire transfer set up.
